Just a heads up regarding the latest version of the app. It doesn’t provide cross device syncing yet, but it allows you to sign in to your Google Drive account and have all your data automatically backed up to make sure they are safe at all time. It also makes restoring the data on a new device really easy.
You can go into Settings/Backup to enable this and sign-in

EDIT: a 1st beta will be available by the end of the day.
FYI the Custom playlists will soon be improved.
Filters will be available so the selected category can only show episodes based on their download status, played status, stared status, publication date, …
I get it that some of you want multiple playlists were episodes can be manually added, but there’s no plan to implement this for now.
However, according to the feedback I got by email, it looks like those filters will solve limitations of the current solution for most users.
Feel free to send me some feedback by email if you have ideas for other filters that can be implemented (Settings/Feedback/Email)

I completely agree with this. I understand that you can make it so that you can download episodes and they get added to the playlist, but I am like you. I don't always add episodes to my playlist. I will download an episode a lot of times to listen when my playlist is cleared.
Alternatively, maybe add a secondary pop-up to ask whether you want to add the episodes you are downloading to the playlist. This can be toggled in the same location that the current setting is but a new option would be "Downloaded episodes only - pop-up". This way you aren't adding a new button or overloading a menu, but just adding an additional function to the already setup section in the playlist setting menu. :)

So I have to say that this idea is second on the list of top ideas, it has nearly 5,000 votes, and there have been comments constantly on it since the start of it until current day. However, I find it funny that the developer is spending so much focus on things with Android Auto (which as a note was suggested in April 2015 and completed 7 months later) but yet this idea has been requested since June 2013 and it says it is started, but 6 years later and we are still waiting. C'mon developer, please give us an update on where you are with this feature. I am sure I speak for us all when I say that it feels like you have forgotten about us and this request or that you just don't care about it.
